These services are designed to continue the progress made by home care and keep patients living independently.

  • Home Safety Assessment
  • Wheelchair Assessments
  • Need for Durable Medical Equipment
  • Need for Caregiver Education
  • Difficulty with Bathing, Dressing, Grooming
